Day Centers offering post stroke care will design a care plan that oversees post-stroke risks like hypertension, stress and depression. That plan will include nursing care to monitor on-going health concerns as well as an exercise program designed to regain or maintain functional abilities such as eating, dressing, bathing, toileting, walking.
